Merger of South Lewisham Group Practice systems
A reminder that South Lewisham Group Practice is merging into Modality Lewisham on 10th June. From noon onward on 10th June until 8am on 15th June we will be unable to offer the following services:
Routine medical appointments
Future appointment booking
During this time we will not have access to your medical records. We will still be able to provide urgent care which cannot wait until our system is live again from 8am on 15th June. Please do not call or visit the practice if your need is not urgent as we will not be able to accommodate routine requests. If you use the NHS App you will not be able to access your medical records including your Covid pass during this time. If you require your Covid pass in the next month, please download it now.
Thank you for your patience during this period of transition. Please note that from 15th June the practice will be known as Modality Lewisham, the practice formed by merger of Bellingham Green Surgery, South Lewisham Group Practice & The Jenner Practice. No members of staff are losing their jobs and you will still be able to see your preferred GP or nurse.

We are encouraging all our patients to use the FREE ‘AskFirst App’ to access useful medical advice with the symptom checker. You can book telephone appointments with your nurse, with a physio or with mental health specialists and book an appointment at GPEA (extended Access).If you haven't already, please download it from your app store.

The surgery uses Doctaly assist to monitor monitor your blood pressure/asthma/COPD/mental health remotely.
The service has been created by the NHS and Doctaly to help monitor patients at home who have certain long term conditions such as hypertension. It is designed to help NHS healthcare professionals safely manage and monitor these patients without the patients needing to physically visit a GP practice. Many of these patients are particularly at risk from COVID-19, therefore home monitoring can help keep them safe.
This practice works with LGBT foundation's Pride In Practice to ensure we provide and meet the needs of our LGBT patients.
